This course on Incident Investigation & Cause Analysis teaches you how to determine, analyze, and prevent incidents at a workplace.

Gather and record, analyze, and document incident evidence. Use and analyze the 5-whys and fishbone diagram. Conduct and witness interviews to collect evidence. Create investigation summary reports, and actioned results. Develop and implement root cause analysis (RCA) to block incidents from recurring. Encourage safety continuous improvement at the workplace.
